NATSIFAC Program - Relocation and settlement of residential care clients into newly completed NATSIFACP facilities
Overview
The purpose of this grant opportunity is to provide funding to assist with the relocation, orientation and settlement of residents into newly built NATSIFACP aged care homes in a culturally safe manner and consistent with requirements of the Aged Care Quality Standards (the standards). The objectives of the grant opportunity are to support eligible applicants with: coordinated relocation logistics such as the development and implementation of plans, procedures and protocols (including critical Chain of Custody for Schedule 8 medications) to ensure a seamless relocation of the service and existing aged care residents in a manner that meets the requirements outlined in Aged Care Act 2024 (the Act), the Aged Care Rules 2025 (the Rules), the Standards (including Outcome 7.2: Transitions), and other Australian and/or State or Territory legislation. temporary engagement of specialists and additional qualified staff, including additional Aboriginal and/or Torres Strait Islander health workers to ensure and support culturally safe, trauma-aware and healing-informed care and processes throughout the transition, and short-term hire of manual labour, additional vehicles and/or necessary equipment for departing and/or receival facilities to meet care requirements under relevant legislation (including occupational health and safety regulations) and to mitigate associated risks for all persons involved. The intended outcomes of the grant opportunity are to: ensure aged care residents experience a safe and well-coordinated transition to the new home and are settled in a manner that prioritises the physical, emotional and cultural care needs of the individual and meets the requirements outlined in applicable legislation, and support a seamless transition of the NATSIFACP service from their old building to the newly constructed aged care home.
