Hospital to Home Program
Overview
This grant opportunity was announced as part of the 2025-26 Mid-Year Economic and Fiscal Outlook (MYEFO). The purpose of this grant opportunity is to extend and build on the success of the previous funding (GO5718) for the Hospital and Home Program (Program). The Program provides ongoing, targeted support to women and families impacted by stillbirth, neonatal death or sudden unexpected death of an infant. Dedicated peer support workers support families within three months of their loss and provide six sessions over six months. The objectives of the grant opportunity are to: • provide early and targeted culturally responsive support to assist in the transfer of bereaved families between hospitals and support services • increase the number of referrals received into the Program from maternity hospitals, hospices, funeral directors or via self-referral • increase the provision of support services for First Nations and multicultural families through the implementation of culturally safe bereavement practices • increase the delivery of culturally responsive support services to rural and remote families, through the training and coordination of bereavement outreach support workers and increased use of telehealth. The intended outcomes of the grant opportunity are to: • reduce isolation felt by women and families following the loss of an infant and acknowledge the impact of grief • reduce the likelihood of complications with grief that can lead to poor mental and physical health outcomes • support bereaved parents in their transition to day-to-day life following the loss of an infant, by supporting them to integrate back into their communities and return to regular activities such as work • increase the understanding of family, friends and community regarding the needs of bereaved families.
